You really need to go back to the basics. Perform a compression test to make sure that the engine does not have a mechanical issue that needs to be fixed. Low compression will give you these same results, a vacuum leak at the intake manifold can also cause this condition. A stuck or lazy O2 sensor can also cause strange conditions. Start at the beginning and see where it takes you.
